PDF Download Instructions:

In order to view and print your forms, be sure you have Adobe Acrobat Reader (free). If you don’t already have this software, you can download it.

Depending on your browser, your selected form will either open immediately, or instruct you to save the file to your hard drive. If the form opens in your browser, you can save it to your hard drive by selecting “Save As” under the “File” menu. You can print a form from the browser by selecting “Print” under the “File” menu.
